Microneedling Aftercare: What to Do Day by Day

Microneedling aftercare means 24 to 48 hours with no makeup, sun or heavy workouts, plus gentle cleansing and daily SPF. Your skin looks like a mild sunburn for 24 to 72 hours, then flakes lightly for a few days. Sunscreen every day is the one rule every clinic agrees on.

Below is a plan for each of the first 14 days and a tap-to-open guide to what is normal. Our licensed estheticians do every microneedling treatment in Fenton with FDA-cleared devices.

What to do after microneedling, day by day

DayWhat your skin doesWhat to doWhat to skip
Day 0 (treatment day)Pink to red and warm, tight, like a mild sunburn. Numbing cream wears off in the first hours.Use only what your esthetician gave you. Drink water. Sleep on a clean pillowcase.Makeup, sun, workouts, hot showers, touching your face.
Days 1 to 2Redness fades over 24 to 72 hours. Skin feels tight and dry.Wash with a mild, fragrance-free cleanser and pat dry. Use plain moisturizer. Wear SPF 30 or higher every day.Makeup and heavy workouts for 24 to 48 hours. Saunas, steam rooms, hot tubs and pools.
Days 3 to 5Dry, rough, light flaking. Tiny white bumps or a mild breakout can show up.Keep moisturizing. Light makeup is fine if your esthetician says so. Light exercise if skin is not tender.Picking, scrubs, retinol, acids, vitamin C.
Days 6 to 7Mostly settled. Some dryness may stay.Keep SPF daily. Ask when to restart your normal products.Restarting actives before you are cleared.
Days 8 to 14Looks like your normal skin. A glow starts to show in 1 to 2 weeks.Keep SPF daily and wear a hat in strong sun. Restart actives when cleared.Tanning beds and long hours in direct sun.

Drink water through the whole two weeks. Healing skin needs it, and it costs nothing. Good aftercare cannot guarantee a result, and it cannot replace advice from your provider. Having a chemical peel as well? Read our chemical peel aftercare plan.

What to expect after microneedling: is this normal?

Tap a color to open it, then tap a line to see what it means.

NORMAL Expected while healing
Red, warm skin on days 0 to 3, like a mild sunburn

NORMAL. This is the expected response. It usually fades in 24 to 72 hours.

Some people stay pink for up to five days. Call us if the redness is still strong after five days.

Tight, dry or rough skin

NORMAL. Common for the first four days. Use a plain moisturizer.

Skin can feel like fine sandpaper around day 3.

Light flaking or peeling

NORMAL. Common on days 3 to 7. Let it come off by itself.

Picking can scar, and it moves bacteria from your hands to your face.

WATCH Mention it to us
Tiny white bumps or a mild breakout

WATCH. Small white bumps (milia) and a light breakout can appear in the first week.

Do not squeeze them. Mention them at your follow-up if they stay.

Pinpoint bleeding, spotting or light bruising

WATCH. Spotting on treatment day and light bruising can happen.

Call us if bleeding goes on past 24 hours.

Swelling

WATCH. Mild puffiness in the first two days is common. Sleeping with your head raised helps.

Call us if swelling lasts longer than one week.

CALL Call us or your doctor
A tingle or burning like a cold sore starting

CALL. Microneedling can set off a cold sore flare-up.

Tell your esthetician or doctor early. Do not wait for it to spread.

Blisters, pus, a honey-colored crust, redness that spreads, a fever over 100 F, or pain that gets worse after day 3

CALL. These can be signs of infection. Call us or your doctor the same day.

Do not cover it with makeup or wait it out.

General information only, not medical advice. If something worries you, call the spa at 1-636-492-2668 or your own doctor.

Why does microneedling aftercare advice differ?

Aftercare sheets do not agree. We compared several clinic guides and Cleveland Clinic. The ranges below are why a friend or a website may tell you something different.

On a small screen, slide the table sideways if a column is cut off.

QuestionWhat guides sayOur spa
MakeupFrom the next day up to day 3 to 5.None for 24 to 48 hours.
Washing your faceFrom 4 to 6 hours after, to the next morning. Some doctors say wait on day 0.Gentle cleanser only, as your esthetician tells you.
Workouts and sweatingFrom 24 hours to 72 hours. No heavy workouts for 24 to 48 hours.
Retinol and acidsFrom 5 days to over a week.Skip them until your esthetician clears you.
Hot showers and saunasFrom 24 hours to the first week.Skip them while skin is tender.
SunscreenSPF 30 or higher daily, for about two weeks. Every guide agrees.SPF daily.

The right answer depends on how deep your treatment went and on your skin. That is why the sheet from your own provider wins over any website, including this one.

What to put on your face after microneedling

Buy before treatment dayFragrance-freeMineral SPF 30+

Buy these before treatment day, so you have them ready when you get home.

  • A mild, fragrance-free cleanser
  • A plain moisturizer with no fragrance
  • A mineral sunscreen, SPF 30 or higher, with zinc or titanium dioxide
  • A hyaluronic acid serum with no other active ingredient
  • Clean pillowcases and towels for the first week
  • A water bottle you will actually finish

Put these in a drawer for at least the first week: retinol, glycolic or salicylic acid, vitamin C, scrubs, facial brushes, jade rollers, gua sha tools, scented products and alcohol toners. The FDA warns that retinol, glycolic acid, menthol, capsaicin and alcohol can irritate skin that has just been treated.

Microneedling downtime: how to plan your week

Stay in 2 to 3 daysNothing big for a weekGym break: 24 to 48 hours

Most aftercare problems start with the calendar, not the skin.

  • Pick a day when you can stay in for the next 2 to 3 days.
  • Keep the week free of weddings, photos and big events. Redness can last up to 5 days.
  • If you wear a mask or helmet all day at work, book before days off.
  • Skip the beach or pool trip in the same week.
  • Plan a break from the gym for 24 to 48 hours.
  • Fall and winter sessions are easier, because the sun is lower. Wear SPF daily in every season.

Microneedling healing process: what happens under the skin

Redness: days 0 to 3Flaking: days 3 to 7New collagen: weeks 2 to 6

Microneedling makes tiny channels in the skin. Your body treats them as a small wound and starts to repair them. Tap a stage to see what to do.

1Days 0 to 3
Response

What happens: Blood and healing cells reach the area.

You see: Redness and warmth.

Do this: Cleanse gently, moisturize, wear SPF and keep your hands off your face.

2Days 3 to 7
Renewal

What happens: The top layer of skin renews itself.

You see: Dryness and light flaking.

Do this: Keep moisturizing and let flakes fall off on their own. No picking, no scrubs.

3Weeks 2 to 6
Rebuilding

What happens: The skin builds new collagen and elastin.

You see: A glow, then firmer skin.

Do this: Keep SPF daily and wait for your esthetician to clear you before restarting actives.

Collagen remodeling takes 4 to 6 weeks. Sunscreen and gentle care protect that process. A sunburn or a picked scab can cause dark patches or scars while the skin is rebuilding.

When does your skin show results?

Results depend on your goal. The FDA has cleared some microneedling devices for facial acne scars, facial wrinkles and abdominal scars in adults 22 and older. It also says results may be temporary and may need repeat treatments. For a goal-by-goal look at the evidence, read does microneedling work.

Who should check first before microneedling?

Tell us your health historySome conditions mean wait

Tell your esthetician your full health history before treatment. Some conditions mean you should wait or skip it.

  • Active acne, infections or open wounds on the area
  • Frequent cold sores
  • A history of keloid scars
  • Blood thinners, a bleeding or clotting disorder, or certain acne medications
  • Eczema, psoriasis or rosacea flare-ups
  • Pregnancy or breastfeeding, depending on the device
  • Chemotherapy or radiation

Our consultation covers your goals and skin history first, and the plan is built from that.

At-home microneedling and derma rollers: not the same tool

Medical-grade: FDA-regulatedHome tools: not regulated

At-home microneedling is sold as a derma roller or a microneedling pen. It is not the same as a professional session.

The FDA says medical-grade microneedling devices are regulated, while short-needle home tools made for exfoliation are not.

Professional microneedling
Medical-grade devices, regulated by the FDA
FDA has cleared some for facial acne scars, facial wrinkles and abdominal scars (adults 22 and older)
Done by a trained provider. The FDA advises a new needle cartridge for each client
Planned around your health history and skin
At-home rollers and pens
Short, blunt needles made for exfoliation
Not regulated by the FDA
Not a substitute for a professional treatment
Do not use on healing skin. Ask your esthetician first

Do not use a home roller or pen on healing skin. Ask your esthetician before you start one again.

Microneedling aftercare questions, answered

These are questions the day-by-day plan above does not answer.

What helps with swelling or discomfort after microneedling?

A cold compress can ease swelling and discomfort, according to a dermatology review in PMC. The same review suggests over-the-counter anti-inflammatory medicine such as ibuprofen for redness and swelling, but some clinic sheets say to skip it. Ask your esthetician first, and tell them if you take blood thinners.

Why does my skin sting when I put on moisturizer or sunscreen?

The FDA says treated skin can sting or itch when you apply cosmetics, moisturizers or sunscreen. A rash, itching or hives can point to an allergic reaction, particularly to numbing cream or serums. If a rash or hives show up, stop the product and call us or your doctor.

Can microneedling cause dark spots or scars?

Yes, but not often. The FDA lists dark or light spots as a less common risk, and a dermatology review in PMC says they usually fade within a few weeks, most often in darker skin types. Lasting scars are uncommon, and daily sunscreen keeps dark spots from getting worse.

Is my skin healed when the redness is gone?

Not fully. Healthline says healing can continue under the surface of your skin for up to 2 months, so skin can look normal while it is still rebuilding. Keep wearing sunscreen, and ask your esthetician before you restart strong products.

What should I ask a provider before my first session?

Ask if you are a good candidate, what the benefits and risks are, and how the device is cleaned between patients. The FDA also says to ask whether a new needle cartridge is used for each patient and each session, and how to care for your skin afterward. Tell your provider about any allergy to numbing medicine.

Does microneedling hurt?

Most people feel warmth or a scratching feeling once it starts. It can feel stronger near bony areas such as the cheekbones. Cleveland Clinic says providers apply a numbing cream such as lidocaine gel about 30 to 45 minutes first, and deeper treatment may cause a little bleeding.

Compare microneedling with chemical peels, see what the research says about red light therapy for skin, or read more about Sarah Hankammer, who owns the spa.
